Age

Any age before age 65 when you terminate due to Total and Permanent Disability

 

Service & Other

  1. 10 Pension Credits (without or following any Permanent Break in Service); and
  2. 300 Hours of Pension Credit in the 36 months immediately preceding application for Disability Pension
 

Basic Benefit

Accrued Monthly Normal Retirement Benefit. Payments begin following a 6-Month Waiting Period.

After attainment of age 65: Benefits continue at same level regardless of disability status.

 

Effective Earlier Of

  1. 1st of the month following submission of application or

  2. 6th month of disability, provided a copy of the social security disability award is provided to the Plan Office within 60 days.
 

Forms of Payment

  1. Single Life Annuity
  2. 50% Joint and Survivor Annuity*
  3. 66-2/3% Joint and Survivor Annuity*
  4. 100% Joint and Survivor Annuity*

    *with or without Pop-up Benefit
    Disability Reduction Factors;
    All forms of payment except the Single Life Annuity are reduced.
