Effective 8/1/11: the benefit increases to $50,000 ($100,000 for accidental deaths). The death benefit does not apply to deaths occurring as a result of suicide or self-inflicted injuries, or upon the death of pre-age 62 retirees who are covered under the Plan as a result of permanent disability. |

Eligibility |
Active participants who satisfy both the minimum hours worked rule and the continuous employment rule are eligible. Application and proof of inability to work satisfactory to the Plan is required.

|
Minimum Number of Hours Worked |
To satisfy this requirement, an applicant must meet at least one of the following employment conditions:
- 250 hours during the 3 months immediately preceding disability
- 500 hours during the 6 months immediately preceding disability
- 750 hours during the 9 months immediately preceding disability
- 1000 hours during the 12 months immediately preceding disability

Continuous Employment |
You must have been employed in Covered Employment for the 36 months prior to onset of disability. For purposes of this rule, months of unemployment during which you were registered with IBEW Local 6 as available for work, provided you were not employed elsewhere, count for eligibility. Apprentice exception: if an apprentice began covered employment less than 36 months prior to disability, continuous employment for the entire period satisfies this rule. |

Limited Benefits Rule |
Participants having less than 36 months may qualify for one month of disability benefits for each calendar month they worked a minimum of 120 hours (or were available for work and registered as such with IBEW Local 6) within the 36 months prior to disability.

Duration of Benefit Payments |
Disability benefits are available for 12 months. After 12 months, must have been awarded a Social Security Disability Benefits. A six month extension to the first 12 months will be granted if a participant has applied for and is awaiting response to application for Social Security Disability Benefits. |

Amount of Benefit |
$400 per month (first 12 months, FICA taxes will be withheld. Benefits are prorated for any month in which the participant is not disabled for the entire month. In addition, $31.25 per month (or prated amount) is payable to the Northern California Electrical Workers Pension Plan. Exception: the pension contribution ceases upon retirement under the Northern California Electrical Workers Pension Plan. |

Waiting Period
|
The first check is payable at the end of the month following a 30 day waiting period, and will resume, thereafter, at the end of each month during period of entitlement.

Timely Filing Requirement |
Application must be filed no later than 90 days after the later of 1) the end of the 30-day waiting period or 2) exhaustion of your health & welfare reserve hourbank balance.
