F. ORGANIZATIONS THAT RECEIVE PREMIUMS
Effective 10/1/2009: As a result of the acquisition of Longs Drug Stores by CVS, the RXAmerica drug card program was replaced with the CVS Caremark program. All references on this page to RxAmerica have been changed to CVS Caremark.

Effective 11/1/2008: the PPO Network will change from First Health to Blue Shield Shared Advantage Program. This change will improve the operation of the indemnity plan with no disruption to Plan participants.
Please note that, as part of this change, all references to the Indemnity Plan in the SPD have been amended to refer to the Self-Funded PPO.
The following organizations receive premiums from the Plan to provide all Plan benefits:
- Union Labor Life Insurance Co. (Stop Loss Coverage - Self-Funded PPO)
- PacifiCare of California (HMO) [replaced by the Blue Shield HMO effective 8/1/2008]
- Kaiser Foundation Health Plan, Inc. (HMO)
