H. DISAPPEAR/RETURNED MAIL.

If the Plan Office is unable to locate you or, in case of your death, your beneficiary for five years (for example, your annual statement is returned in the mail and the Union does not have your address), the Plan may close your account; however, if you (or your designated beneficiary) later appear, the Plan will reinstate the balance of your individual account at the time the individual account was closed. No earnings or expenses are allocated to the account after the account is closed.

If the Plan has been unable to locate you or your Beneficiary entitled to death benefits from the Plan within five (5) years, the unclaimed benefits will be applied to the expenses of the Plan and you or your Beneficiary’s entitlement to benefits will be reinstated if and when you or a lawful beneficiary subsequently presents a valid claim to the Plan Office. Any such reinstated benefits will not include earnings that may have accrued after the unclaimed account was closed and allocated to Plan expenses.
