B. APPLICATION AND PAYMENT OF BENEFITS.
To receive your benefits, you should file a written application in a form and manner prescribed by the Plan within 90 days of your anticipated retirement date. Applications may be obtained from, and should be submitted to, the Plan Office at 720 Market Street, Ste. 700, San Francisco, California 94102 All of the retirement forms are available on the EISB.org website.
Your application should include a copy of your birth certificate or other legal document verifying your date of birth and, if married, copies of your marriage certificate and your spouse’s proof of birth. You should also submit any divorce documents relating to a prior spouse or settlement agreement relating to a prior registered domestic partner, including the Final Judgment and any Settlement Agreement that grants, or reserves the right to grant, a spouse, former spouse, domestic partner, child or other dependent a portion of your benefits.
Benefit payments are effective the first day of the month following the date you file your completed retirement application and you are eligible to receive your benefits. Benefit checks are prepared the first day of each month and will commence as soon as it is administratively feasible after all contributions are received and your application is processed. Thus, filing a timely application is important.
